We understand that the finding the right level of care can feel confusing and overwhelming. We hope that his page will provide an initial guide to finding the right level of care custom to your needs.

Skilled Nursing Facilities

A nursing home is normally the highest level of care for older adults outside of a hospital. Nursing homes provide what is called custodial care, including getting in and out of bed, and providing assistance with feeding, bathing, and dressing. A licensed physician supervises each patient’s care and a nurse or other medical professional is almost always on the premises. Many skilled nursing facilities offer nursing care in one convenient location 24 hours a day, 365 days a year. Depending on the skilled nursing facility occupational, physical, and speech therapists, are also available. If a patient needs rehabilitation, a patient receives the services until they’re able to go home.
The label “nursing home” has negative connotations for many people. Yet nursing homes provide an important component of senior housing options.

Living arrangements in a nursing home

The environment and layout varies from the nursing home to nursing home. For example, some rooms may be private and others shared. Rooms may have their own bathroom, or bathrooms may be shared.

Short-term & Long-term Care Available

Short-Term Care

If you are seeking short-term medical and/or rehabilitative services, we will focus our efforts working closely with you and your family on achieving the most successful functional outcomes so that you can return home as quickly as possible. A dedicated team of healthcare professionals will design an individualized care plan that is tailored to your specific needs. Supporting the nursing team is a group of licensed therapist who provide physical, occupational and speech therapies for patients recovering from major surgery, stroke, neurological and orthopedic conditions and other illnesses, injuries and disabilities. We are guided by the principle that quality healthcare which is our mission is best achieved when we work together.

Long-Term Care
Our long-term care units are designed for individuals who need 24-hour nursing care and may chronically ill, frail or experiencing a very slow recovery from an illness or injury. These units offer both semi-private and triple room accommodations. Our visitors are welcome to enjoy visiting with residents in our activity room or our lovely outdoor patio courtyard. In addition, we offer respite or vacation care that allows families an opportunity to take a brief break from the demands of caring for a loved one round-the-clock.
Skilled nursing facilities provide an extensive range of care under the supervision of licensed nurses and doctors who specialize in the care of the elderly. The following is a list of care that may be offered by skilled nursing facilities.

  • Stroke Recovery – helping patients regain motor skills and speech functions.
  • Parkinson’s Care – specific care required to attend advanced Parkinsons’s Cases.
  • Custodial Care – helps patients with ADLs.
  • General Wound Care – cleans wounds and administers antibiotics for infection.
  • Acute Medical Conditions – assistance where an illness, infection or injury is present.
  • Terminal Illness Care – services offered along with hospice care for people living with terminal illness.
  • General Rehabilitation speech, physical, and occupational therapy and other services to help patients recover after an extended illness or invasive operations.

Residential Care Facilities

Assisted living facilities are group living environments that provide assistance with some activities of daily living (ADLs). Whether they provide nursing services or help with medications varies among states. We recommend that you contact assisted living facilities to see if they can perform all levels of care that are necessary for your best well-being.

In assisted living, people often live in their own room or apartment and have some or all of their meals together with other residents. Social and recreational activities are usually provided.

These facilities provide help with ADLs. Some help with care most people can do themselves (like taking medicine, getting to appointments, or preparing meals).

Hospice & Respite Care

Hospice is a program of care and support for people who are terminally ill. Hospice helps people who are terminally ill live comfortably. When you choose hospice care, you’ve decided that you no longer want care to cure your terminal illness and/or your doctor has determined that efforts to cure your illness aren’t working.

Respite care is a very short inpatient stay given to a hospice patient so that their usual caregiver can rest.

Home care:

Depending on your needs, you may be able to get help with your personal care and ADLs at home from family members or friends. Home care agencies are also available to help with personal care, like bathing and help walking.

Resident and Team Member Covid-19 Status Update as of 01/16/2023

 

The information below is Empress Care Center’s regular update to help ensure residents, family, team members and the community are kept up to date on our current COVID-19 status.

In the event of a positive case, we have protocols in place to protect our residents and team members including testing and immediately isolating residents with suspected or confirmed COVID-19. In addition, team members with suspected or confirmed COVID-19 are tested and directed to stay at home until they fully recovered.

COVID-19 positive residents: 0

Three or more residents or staff with new-onset of respiratory symptoms occurring within 72 hours of each other: 0

Team members currently unavailable to work because of a COVID-19 positive test: 2

Team members who have recovered: 18

 


Empress Care Center Visitation Update

When visiting Empress Care Center, as part of our updated screening process as required by CDPH’s State Order, , all visitor regardless of their vaccination status, must adhere to the following guidelines:

  • All visitors must be screened for fever and COVID-19 symptoms and/or exposure within the prior 14 days to another person with COVID-19; if a visitor has COVID-19 symptoms or has been in close contact with a confirmed positive case, they must reschedule their visit, regardless of their vaccination status.
  • All visitors must wear a well-fitting face mask with good filtration (N95, KF94, KN95, or surgical masks are preferred over cloth face coverings) and perform hand hygiene upon entry and in all common areas in the facility;
  • If personal protective equipment (PPE) is required for contact with the resident due to COVID-19 positive isolation status (including fully vaccinated visitors), it must be donned and doffed according to instruction by HCP.
  • Facilities should limit visitor movement in the facility, regardless of the visitor’s vaccination status; for example, visitors should not walk around the hallways of the facility and should go directly to and from the resident’s room or designated visitation area.
  • CDPH allows for only one exemption to the order: visitors to a patient in critical condition, when death may be imminent.

Visitors who have tested positive for COVID-19 should not be permitted to visit or should be asked to leave if they are still within their isolation period (within 10 days of their positive test). Under such circumstances, facilities must offer alternatives for remote (Skype, etc.) or telephone visitation. Staff should provide monitoring for those who may have difficulty adhering to core principles, such as children

Disclaimer

At this time, Empress Care Center is not releasing medical records to discharged residents or their legal representatives in person due to the current COVID-19 pandemic Public Health Emergency. However, we do offer other options for obtaining medical records.

Discharged residents may request a copy of their medical records verbally and/or by completing and submitting an Authorization for Release of Personal Health Information form.

You can download and complete the authorization form from our website to submit your medical record request by fax, email or mail. A Medical Records representative can also email you a form to compete, fax, email or mail back to the Center.

Verification of identity may be required. Acceptable forms of identification include driver’s license, state-issued ID, or a valid, current passport.

If you are requesting for copies of medical records, standardized copying charges will be applied in addition to any postal charges or CD ROM charges. Records will only be released via mail or picked up in person with an advanced appointment.

The Medical Records Department’s hours of operation are Monday – Friday, 8:30 a.m. – 5 p.m. The department is closed on weekends and major holidays. Please direct your inquiries to the Med Rec department for status updates or the Center Administrator.

Due to the current PHE, CMS modified the requirement at 42 CFR §483.10(g)(2)(ii) to allow LTC facilities 10 working days to provide a resident’s record rather than the standard two working days. If the requested information is located off-site or if the authorization form is not properly filled out, additional time may be required to process your request.
If you are requesting copies for someone other than yourself, you will need to provide legal documentation that verifies legal guardianship, power of attorney, executorships or next-of-kin relationship to a decedent.
