As a retired registered nurse with the Department of Public Health, I have been involved in in-depth investigations regarding quality of care at nursing homes throughout several counties. I have found Kim, the Director of Nursing at Empress, to maintain the highest quality of care for all residents (patients). She continually strives to ensure her staff follow her high standards of care. If I ever have a concern, Kim and her wonderful staff take the time to answer all my questions and investigate my concerns. The staff and the Doctor listen to my concerns and make changes if warranted. Each of my concerns is taken seriously. This is why they maintain a 5-star rating.

When I was choosing a nursing home for my Aunt who has had Alzheimer’s over 20 years, my first choice was Empress Care. I knew from my previous interactions as a Department of Health investigator, my Aunt would have great care.

They have improved my Aunt’s life greatly ensuring she walks several times a week with staff. She was a walker her whole life, but then became bedridden with her last caretaker, so to see her waking again is wonderful. Whenever my Aunt states she needs to use the bathroom, staff bring her immediately to the bathroom and give her the opportunity to try to use the toilet, even though most times she is incontinent.

I am happy with the care of my Aunt, and am no longer worried about her on a daily basis. Thank you to Kim, Bea, and all the caring staff at Empress!

Resident and Team Member Covid-19 Status Update as of 01/16/2023

 

The information below is Empress Care Center’s regular update to help ensure residents, family, team members and the community are kept up to date on our current COVID-19 status.

In the event of a positive case, we have protocols in place to protect our residents and team members including testing and immediately isolating residents with suspected or confirmed COVID-19. In addition, team members with suspected or confirmed COVID-19 are tested and directed to stay at home until they fully recovered.

COVID-19 positive residents: 0

Three or more residents or staff with new-onset of respiratory symptoms occurring within 72 hours of each other: 0

Team members currently unavailable to work because of a COVID-19 positive test: 2

Team members who have recovered: 18

 


Empress Care Center Visitation Update

When visiting Empress Care Center, as part of our updated screening process as required by CDPH’s State Order, , all visitor regardless of their vaccination status, must adhere to the following guidelines:

  • All visitors must be screened for fever and COVID-19 symptoms and/or exposure within the prior 14 days to another person with COVID-19; if a visitor has COVID-19 symptoms or has been in close contact with a confirmed positive case, they must reschedule their visit, regardless of their vaccination status.
  • All visitors must wear a well-fitting face mask with good filtration (N95, KF94, KN95, or surgical masks are preferred over cloth face coverings) and perform hand hygiene upon entry and in all common areas in the facility;
  • If personal protective equipment (PPE) is required for contact with the resident due to COVID-19 positive isolation status (including fully vaccinated visitors), it must be donned and doffed according to instruction by HCP.
  • Facilities should limit visitor movement in the facility, regardless of the visitor’s vaccination status; for example, visitors should not walk around the hallways of the facility and should go directly to and from the resident’s room or designated visitation area.
  • CDPH allows for only one exemption to the order: visitors to a patient in critical condition, when death may be imminent.

Visitors who have tested positive for COVID-19 should not be permitted to visit or should be asked to leave if they are still within their isolation period (within 10 days of their positive test). Under such circumstances, facilities must offer alternatives for remote (Skype, etc.) or telephone visitation. Staff should provide monitoring for those who may have difficulty adhering to core principles, such as children

But, because of the significant risk posed by COVID-19 and the variants, our obligation is to ensure we follow federal, state, and county public health guidelines and keep you connected with your loved ones during these challenging times.

Disclaimer

At this time, Empress Care Center is not releasing medical records to discharged residents or their legal representatives in person due to the current COVID-19 pandemic Public Health Emergency. However, we do offer other options for obtaining medical records.

Discharged residents may request a copy of their medical records verbally and/or by completing and submitting an Authorization for Release of Personal Health Information form.

You can download and complete the authorization form from our website to submit your medical record request by fax, email or mail. A Medical Records representative can also email you a form to compete, fax, email or mail back to the Center.

Verification of identity may be required. Acceptable forms of identification include driver’s license, state-issued ID, or a valid, current passport.

If you are requesting for copies of medical records, standardized copying charges will be applied in addition to any postal charges or CD ROM charges. Records will only be released via mail or picked up in person with an advanced appointment.

The Medical Records Department’s hours of operation are Monday – Friday, 8:30 a.m. – 5 p.m. The department is closed on weekends and major holidays. Please direct your inquiries to the Med Rec department for status updates or the Center Administrator.

Due to the current PHE, CMS modified the requirement at 42 CFR §483.10(g)(2)(ii) to allow LTC facilities 10 working days to provide a resident’s record rather than the standard two working days. If the requested information is located off-site or if the authorization form is not properly filled out, additional time may be required to process your request.
If you are requesting copies for someone other than yourself, you will need to provide legal documentation that verifies legal guardianship, power of attorney, executorships or next-of-kin relationship to a decedent.
